Privacy statement
Grey Lynn Physio Limited respects your privacy and handles your personal and health information in line with the Privacy Act 2020 and the Health Information Privacy Code 2020.
What we collect and why. We collect the information we need to assess and treat you, such as your contact details, health history and details of your injury. We use it to provide your care, communicate with other health providers involved in your care, and claim funding from ACC or your insurer where applicable. We also use it for quality improvement and audits.
Where it comes from. We collect most information directly from you. Sometimes we receive information from others, such as your GP, a specialist or ACC. If we do, we'll let you know.
Who we share it with. We only share your information with people directly involved in your care (for example your GP or referrer), with ACC for ACC claims, or with your insurer if you ask us to. We won't share it with anyone else, such as your employer or a lawyer, without your consent, unless the law requires or allows it.
How we keep it safe. Your records are stored securely in our practice management system (Gensolve), protected by passwords and security controls. Your health information is stored in New Zealand or Australia. We keep health records for at least 10 years, as required by law, and then dispose of them securely.
Your rights. You can ask to see your health information or to have it corrected at any time. We'll respond within 20 working days. There's no charge to access your own information.
